North Core Medical offers in-home physician visits for established members whose illness, limited mobility, disability, recovery from surgery, or serious health condition makes an office visit difficult or unsafe.
Home visits provide the same personalized, relationship-based care you receive in the clinic. Your physician can assess appropriate concerns, review medications, monitor chronic conditions, provide supportive or palliative primary care, and coordinate additional services when needed.

Our approach
Before scheduling, we will review your needs to confirm that a home visit is medically appropriate. Some examinations, testing, imaging, procedures, or treatments may still require care at the clinic or another medical facility.
In-home physician visits are not included in the standard Direct Primary Care membership fee. The additional fee may vary based on distance, travel time, and visit complexity. Pricing will be explained before the appointment is confirmed.
In-home physician visits are scheduled primary care appointments. They are not an emergency, urgent-response, ambulance, or 24-hour home-care service. For a medical emergency, call 911 or go to the nearest emergency department.
